Owner, Lee Berkowitz has been happily surrounded by the health care industry for years. He served an integral role with his family's health care business, worked in an assisted living facility, and in a non-emergency medical transportation company – which further developed his passion for health care. That’s where Welcome Back Home Care was born and Lee has been providing the right care for his clients ever since.
Lee was born in the North Shore Hospital in the city of Manhasset, in New York State. Later, the Berkowitz family moved to Coral Springs, Florida where Lee attended Taravella High school and graduated with a Bright Futures Scholarship in 2005.
Shortly after graduating college in 2012 with a degree in Hospitality Management, Lee knew there was a reward for those who are generous and believe in themselves. With that insight, and a strong support system in motion, the lightbulb flickered: we can apply service, hospitality, and homecare!
Lee was deliberate to write a business plan of action and in 2014, we welcomed the birth of Welcome Back Home Care!
Always enjoying travel: Lee visited the country of Israel on a Birthright Tour in 2013; Lee has visited Colombia twice, and visited a natural coffee farm in the mountains of South America.
In 2019, Lee attained a real estate License.
In 2016, Lee attained a 2-15 License (Insurance)
Favorite Books: The Biography of Benjamin Frankin by Walter Isacson. Awakening a New Earth by Eckhart Tolle.
Hobbies Include: ongoing charitable events, beach clean-ups in Delray Beach and beyond, food drives, watering local gardens, a good book, golf, chess, and an ongoing education, water sports, the beach, and cooking dinner.
Lee is humbly honored to be welcome to serve our local community and the constituents of Welcome Back Home Care.

Jeffrey A. Levin
Jeff was introduced to the world of home health care when his father suffered a head trauma that took him from totally self-sufficient to needing 24-hour attention. … he could resume all normal functions, and only require the presence of a care giver through the overnight hours, which was a precaution to allow him to maintain his independence.
This humbling experience not only taught Jeff the need to plan for the long-term care needs of family members, but also how to take immediate action when no plan is in place.
Jeff has been with his wife Dirce, together for 21 years, and has been mutually adopted into this family that has grown to 3 adult children and 6 grandchildren. He enjoys helping people and animals, believing that life is special, and is so thankful that he has this rewarding career where he can make a difference in people’s lives.

Serving others is Katie's passion. Mrs. Chang joyfully engages in serving as a wife, mother of two beautiful children, and professionally at Welcome Back Home Care. Working in customer service since 2003 and as a Registered Pharmacy Technician prior, Mrs. Chang has additional professional experience that has prepared her well for her role as Human Resources Manager in our Home Health Care agency.
Besides her professional expertise, Katie loves spending time with her family, baking, and reading. We’re honored to have Katie on our team!

Compassion inspires Jen to be an excellent part of the Welcome Back Home Care team, bringing our valued clients reliable billing and supporting services. Jen grew up in Massachusetts and has been living in South Florida since 1998. In 2005, her neighbor owned and ran a home health care business and asked Mrs. Hoffman to join the company. … office manager for a few years, she left to run a business with her husband. Later, she spent 10 years as executive director of a performing arts nonprofit where Jen also performed and taught classes.
In 2018, when her father was caring for her mother, who suffered from dementia, he was diagnosed with cancer. Mrs. Hoffman’s parents were caught unprepared, so she left her position to spend 2-1/2 years caring for her parents and managing their situation. During that time, both parents passed away and she was left with a heavy heart and a deep understanding of some of the experiences that elderly people and their families may go through.
After her mother passed away from Covid-19 in 2020, Jen was contacted by her neighbor’s son, Lee, who owns and runs Welcome Back Home Care, and here she is!
Jen is glad to be a voice on the other end of the phone who empathizes and cares about the situation our clients and families find themselves in.
